The Continuing Care Patient Navigator primarily assists both patients and members of the department and healthcare team, and may work with colleagues across the care coordination continuum. Duties include collaborating with team members to achieve member goals, telephonically assisting members, and other administrative duties. This is a certified MA role working Monday through Friday, with some in person meetings at our corporate office. Must live in CO.
